Requests should be received a minimum of 2 weeks in advance of the service date. Services are scheduled after this form is received in the Multimedia system. For requests received less than 1 week in advance, rush charges will apply. Rush charges are higher the later requests are received. Room reservations should be extended prior to and following the event to allow sufficient time for setup and takedown of equipment.

This request is made in support of university activities. Departments will be charged for any contracted services, materials used, and, when necessary to recover expenditures, for technical personnel. In the event of cancellation, 4 days advance notice is requested, otherwise labor charges may apply. The department below acknowledges full financial responsibility for any costs incurred due to damage or loss of equipment resulting from utilization for this event.

I agree and am authorized to commit the organization represented for the charges related to this service. When this request involves live recording, I understand that it is my responsibility to ensure that the recording as requested will be in compliance with all applicable laws pertaining to the privacy rights of individuals and groups. I certify that this request is made in compliance with the copyright law of the United States.
